Figure 1 Acute guttate psoriasis on arms and trunk. One such trigger is infection, and one of the more common infectious agents is group A -hemolytic streptococcus (GABS). COM) More than 50 years ago, a study showed that in a series of 32 patients with acute guttate psoriasis (AGP; Figure 2), 56 had an antistreptolysin O (ASO) titer of 200 or more, and 31 had a history of sore throat 1 to 3 weeks before the appearance of the rash.7. Although the association between streptococcal infection and AGP is perhaps the most well described,8,10 strep infection has been shown to be associated with plaque11 as well as with pustular psoriasis.4. Guttate psoriasis is a type of psoriasis that looks like small salmon-pink drops on the skin. However, some people with guttate psoriasis develop the most common form of psoriasis, called plaque psoriasis. The trigger for the condition is usually a streptococcal (bacterial) throat infection. Small, salmon-pink (or red) spots usually appear suddenly on the skin two to three weeks after a streptococcal infection, such as strep throat or tonsillitis. Guttate psoriasis is characterized by the acute onset of small, 1-10 mm diameter, droplike, erythematous-to-salmon-pink papules, usually with a fine scale, as demonstrated in the images below. Guttate psoriasis is more common in individuals younger than 30 years. The sudden appearance of the papular lesions in response to streptococcal infection could either be the first manifestation of psoriasis in a previously unaffected individual or an acute exacerbation of long-standing plaque psoriasis. Lotus et al demonstrated that guttate psoriasis patients expressing the HLA-Cw 0602 allele were twice as likely to have positive streptococcal throat cultures.

Psoriasis, especially guttate forms, may be provoked or aggravated by streptococcal infection. Streptococci pneumoniae (pneumococci) are bacteria important in pneumonia and meningitis but rarely cause skin disease. Guttate psoriasis can cause numerous smaller lesions that have the same features as those of plaque psoriasis. For example, if you are infected with a streptococcal infection, the immune system generates antibodies to counter streptolysin-0 which is present on the bacteria causing the infection. Upper respiratory and strep throat infection are common, but not everyone suffering from them contracts guttate psoriasis. Guttate psoriasis is most often triggered by upper respiratory infections (for example, a sore throat caused by streptococcal bacteria).
